The downloadable 2019 GPCA NS Premium List does NOT contain an entry form — follow the instructions below.
To submit an email entry you must use the 2019 GPCA NS BaRay Entry Form. You can fill out the form on the computer by downloading and opening the file, then completing the appropriate sections. Apply an electronic signature to the completed form.* Save the completed form and email to email@example.com. For credit-card payments follow payment instructions on the entry form. NOTE: GPCA does not recommend sending credit-card details by email, as it is not secure.

Download the 2019 GPCA NS BaRay Entry Form and fill out the form as above. Make sure that you sign the bottom of the first page. Per AKC Rules, BaRay cannot accept entries that do not have the AKC agreement on the second page of the entry form. Print the completed form and send by courier, US mail, fax ($4.00 charge per event for faxed submissions), or scan and email the entry to BaRay. Include your form of payment.
